Rebate Requirements:
- Purchase any Cascade ActionPacs product, up to $5.00.
- Submit the original receipt with the purchase price circled, along with the product UPC code and the completed rebate form.
- The rebate cannot be combined with other offers, including coupons.
- Purchase the product before 12/31/14. The offer must be postmarked by 1/31/15.
- Allow 6-8 weeks to receive the rebate in the form of a prepaid card.
- There is a limit of one rebate per household.
